Use this form if you have a problem with a plant that is not thriving, or has an infestation, disease, or disorder. If you have a general gardening question, please use the "General gardening question" form instead.

You cannot include photos with this form. If we need photos, we will contact you by email to request them.

Please submit one plant per form. Please answer all questions on the form. If you cannot answer a question, fill in "not sure", "don't know", or "not applicable". The more information you provide, the faster we can help.

What is the name of the plant you have a question about? If unknown, indicate if it is a tree, shrub, perennial, annual, or lawn, and describe what it looks like.

Describe the problem with your plant. What do you think is wrong?

How old is the plant? When did the problem start?

What part of the plant is affected? What percentage of the plant is affected? Please describe the symptoms in detail.

Did you do anything to treat the problem? If so, what have you tried?

Are other plants in the area affected too?

Describe how the plant has been cared for. How much and how often is the plant watered? How much and how often is it fertilized? How many hours of direct and indirect sunlight does it receive? Is the plant mulched? If the plant is a tree or shrub, is it regularly pruned?

Has anything changed in the area where the plant is growing in the past three years (construction, flooding, drought, gasoline spills)? If so, please describe.

Were any herbicides or insecticides used near the plant? if so, please describe.
